  1. Every act needs a start and an end point. Its needs resolution and frequency to determine the distance. It needs compliance and compliment to establish the depth. The conformance to the goal is possible when both quantity and quality are raised to a better value than the current value. For the aforesaid philosophy, measurements are imperative. "If you can't measure it, you can't manage it" – This thought/ belief drives the requirement of a measuring system for every undertaking involving systems. The performance of any process will be analyzed based on the numbers set to achieve. Learning is not possible if the measurements are not in place. Vision gets blurred as the focal length is not adjusted with regards to the momentum towards the goal. "It is wrong to suppose that if you can't measure it, you can't manage it. The most important numbers are unknown and unknowable" - Well, this belief is a warning that you should not have a predefined set of measurement as it leads the thinking/drive for excellence confined to the boundaries of those numbers. Unexplored are the treasures for new beginnings, opportunities, growth, potentials and learning. When we talk about the concentric circles of Control, Influence and Concern, Unknown area will be ventured only by expanding the circle of influence. Hence by being effectively predictive and strongly proactive one can achieve the unimaginable. Continuous improvement of processes and questioning status quo are the solution areas that will help these gentlemen settle their differences.
  2. Hi All Here is what I interpret of the topic. For an investigation which is aimed at reaching an argument/observation should have this flow Appropriate data collection [ ACCURATE and PRECISE ] Followed by the information derived from it [SIGNIFICANT and RELEVANT] The knowledge extracted of it [NECESSARY and SUFFICIENT] Finally wisdom attained [ WHY/WHY NOT and IS/IS NOT ] Necessary - Lets say it as "a must" condition and Sufficient - is a "good enough" state. In order to attain quality - Reduction of variation is NECESSARY and the SUFFICIENT condition which convinces us that we are on right track is adoption of identifying and tackling 7+1 wastes. Hence an effort has to be both necessary and sufficient ( Provided that the system is robust { insensitive to violations of assumptions}). Compiling both sort of conditions and their impact on corresponding negligence should be carried out with utmost criticality. Extending the discussion onto the RCA/Problem solving domain - I better be sure of my analysis and various components of my synthesis to be Mutually Exclusive Collective Exhaustive with Acknowledged Assumptions. For these cases A cause may be necessary but not sufficient for a problem to occur - Chase it and dissect further A cause may be sufficient but not necessary for a problem to happen - Dont just ignore it. Assumptions are costly. A cause may be neither sufficient nor necessary for a problem to occur - Why is it in the list. Can I reverse think of it and check if its needed for my analysis A cause may be both sufficient and necessary for a problem to occur - Hmmm... does it talk. Am I listening. Is it a decoy.   5. Dear Rajiv Hope this helps if iam not late. When we visit "Subway" for a roll. We have these chefs work on each roll different depending on the request. Well the CTQs are similar in all of their delivery, yet the taste varies depending on the activities while making the roll. If he adds too many vegetables, or sausages or the meat or underheat the bread - the accuracy with the taste falters. I find your challenge similar to a typical manager's worry on ensuring the right taste of each roll as per their brand( in your case the SLAs ) What I can think of is if we break the activities of these cases as they have similar CTQs into segments( sequential or parallel )/ work blocks and check for the accuracy ( based on the criteria set for each segment) on a random basis and then extrapolate the entire work - we might have an answer. I can also recommend the structure followed by the famous " Dabbawalas" of Mumbai. They break it down into stages, track it and ensure the delivery to the right candidate.
