Poka yoke / Mistake Proofing
I suppose the 4th option i.e. 'Human error may happen, the defect will also happen but will be detected and corrected automatically. ' is most appropriate interpretation for poka-yoke. Whenever and whichever process humans are involved, error can't be elimitated in totality. If error happens, defect will happen. Most important is to ensure that the defect is rectified and not allowed to pass to next stage, else this would lead to output being defective. There are several examples of poka-yoke. Would like to site one such example from my field- Circuit breakers - they trip in case of overload current and the current flow stops The breaker don't stop the overload current from reaching the circuit but doesn't allow it to flow through remaining circuit. Thus saving the remaining elements which are mostly high value items. Conclusion- though the defect occurred i.e. circuit breaker tripped, it has saved the remaining elements i.e. defective circuit creation has been avoided.

CTQ / Metric
Dashboards, metrics,data,etc. form the basis of information which help in analysing the problems and hence resolve. Still, as dr. Deming stated the above mentioned, the unknown are meant to be explored even further and more deeper and balanced. Also, there should be no baised thoughts or views while resolving the unknown issues.

Question : Define, compare & contrast the following terms – Personal Excellence, Process Excellence, Operational Excellence and Business Excellence. Answer 1: Personal Excellence is the ability to create solutions in difficult solutions to enable yourself to successfully overcome the problems. This means “self-management”. Process Excellence is improving the way businesses create and deliver value to customer. Operational Excellence is an element of organizational intelligence that focuses on meeting customer expectations while stressing the application of a number of principles, processes and tools towards sustainable improvement Business Excellence is using exceptional practices for managing the organization and achieving the desired results, all being based on fundamental concepts or values. Personal Excellence is managing self and striving to overcome difficulties individually. Process Excellence results in effective and efficient process as a result of personal excellence. It helps improve Operational Excellence of the organization by focusing on developing and delivering consistently near-perfect products, processes and services. Operational Excellence is process excellence with additional focus on cultural and systemic improvements. Business Excellence is the most broader of all since it is for the entire organization, .i.e. the entire organization having best processes delivering outstanding results for all. Hence Personal Excellence is a subset of Process Excellence which in turn is a subset of Operational Excellence which finally is a subset of Business Excellence.
